If you or your ministry is hosting an event, please fill out this form. It will be sent to the St. Alban's Office for review. If approved, you will be notified and it will be added to the church calendar.
Absence Request Form (Staff Only)
Use this form if you are requesting time off for any reason. Any time off (other than sick time) mube be sumbitted at least two days prior to the first day requested. Vacation time must be submitted at least 30 days before requested date. Requests will be reviewed by the Rector.
If you see, hear about, or know of a maintenance need, please fill out this form. Copies will be sent to the Junior Warden and church office, who will follow up if necessary.
If you need to request supplies, materials, or labor/service, or if you need to be reimbursed for either, please fill out this form. A copy will be emailed to the business manager.
