Prayer Request Form

Prayer is an integral part of our lives at St. Alban’s and we want to include your needs and thanksgivings as part of our regular prayer ministry.

Confirmation, Reception, and Reaffirmation Form

This form is for those preparing to be confirmed, received, and reaffirmed in The Episcopal Church.

Event Request Form

If you or your ministry is hosting an event, please fill out this form. It will be sent to the St. Alban’s Office for review. If approved, you will be notified and it will be added to the church calendar. Please refer to Exit Checklist for St. Alban’s Kitchen/Parish Hall for building use requirements.

Absence Request Form (Staff Only)

Use this form if you are requesting time off for any reason. Any time off (other than sick time) must be submitted at least two days prior to the first day requested. Vacation time must be submitted at least 30 days before requested date. Requests will be reviewed by the Rector.

Maintenance Request Form

If you see, hear about, or know of a maintenance need, please fill out this form. Copies will be sent to the Junior Warden and church office, who will follow up if necessary.

Funeral Form

When planning a funeral, please use this form to assist with that process.

Valda McWhirter Memorial Scholarship Application

Through the generosity of Mrs. Valda McWhirter of St. George’s Episcopal Church in Gatesville, St. Alban’s Episcopal Church in Waco is blessed with the opportunity to provide educational scholarships to St. Alban’s members in the amount of $1,000 to $3,000 per academic year (based on available funds). These awards are renewable for up to five undergraduate study academic years (need not be consecutive) or three graduate study academic years, provided the student continues to meet eligibility requirements.
